HSC Medical Center Home Health is committed to the philosophy that individuals in need of skilled nursing care and other home health services should have access to such treatment at home unless there is a compelling medical/social reason prohibiting such care. Home Health provides comprehensive multidisciplinary care, tailored to assist the individual and family to achieve the optimal level of care. We provide care that is based upon respect for the dignity and worth of the individual and family. The Home Health Team utilizes a multidisciplinary approach and includes patients and their families; a patient's private physician; a Home Health nurse; a physical therapist; and an occupational therapist as part of the team. Home Health can also provide a Home Health Aide to assist patients with activities of daily living if needed. Home Health shares a speech-language pathologist and a medical social worker with the hospital. Referrals can be made 24 hours a day, seven days a week. We welcome referrals from physicians, hospitals, rehabilitation programs and other care facilities as well as family members, insurance companies and case managers. HSC Medical Center Home Health provides in home evaluations within 48 hours of a referral.
